Effects of respiratory resistance training with a concurrent flow device on wheelchair athletes.

BACKGROUND/OBJECTIVE: To determine the effect of respiratory resistance training (RRT) with a concurrent flow respiratory (CFR) device on respiratory function and aerobic power wheelchairs, handicap scooters athletes. METHODS: Ten male wheelchair athletes (8 with spinal cord injuries, 1 with a neurological disorder, and 1 with postpolio syndrome), were matched by lesion level and/or track rating before random assignment to either a RRT group (n = 5) or a control group (CON, n = 5). The RRT group performed 1 set of breathing exercises using Expand-a-Lung, a CFR device, 2 to 3 times daily for 10 weeks. Pre/posttesting included measurement of maximum voluntary ventilation (MVV), maximum inspiratory pressure (MIP), and peak oxygen consumption (V(O2peak)). RESULTS: Repeated measures ANOVA revealed a significant group difference in change for MIP from pre- to posttest (P < 0.05). The RRT group improved by 33.0 cm H2O, while the CON group improved by 0.6 cm H2O. Although not significant, the MW increased for the RRT group and decreased for the CON group. There was no significant group difference between V(O2peak) for pre/posttesting. Due to small sample sizes in both groups and violations of some parametric statistical assumptions, nonparametric tests were also conducted as a crosscheck of the findings. The results of the nonparametric tests concurred with the parametric results. CONCLUSIONS: These data demonstrate that 10 weeks of RRT training with a CFR device can effectively improve MIP in wheelchair athletes. Further research and a larger sample size are warranted to further characterize the impact of Expand-a-Lung on performance and other cardiorespiratory variables in wheelchair athletes.

Integrated control and related technology of assistive devices

Assistive devices are now available that allow persons with severe physical disabilities to complete tasks independently. When the user has severe physical limitations, it may be advantageous to have an integrated control system where a single control interface (e.g., joystick, head switches, voice recognition system, keypad) is used to operate two or more assistive devices (e.g., power wheelchairs, augmentative communication devices, handicap scooters, computers, environmental control units, and other devices that are controlled electronically). The advantages of integrated control are that persons with limited motor control can access several devices with one access site without assistance, and the user does not need to learn a different operating mechanism for each device. The purpose of this review is to convey the depth and breadth of the research that has been conducted on integrated control systems, as well as to provide some insights into future directions. We reviewed research works pertaining to communication and environmental control, computer access, and wheelchair guidance systems. Information gathered in this study will help people become fully aware of the status of contemporary integrated control technology in order to increase the quality of life of people who use electronic assistive devices.

Often times the physically challenged, limited to a medicare power wheelchairs, also have difficulty with vision. In order to help, something must “see” for them. Therefore there must be some way for a wheelchair to know its environment, sense where it is, and where it must go. It also must be able to avoid any obstacles which are not normally part of the environment. An autonomous wheelchair will serve an important role by allowing users more freedom and independence. This design challenge is broken into four major steps: wheelchair control, environment recognition, route planning, and obstacle avoidance. The first step is to reverse engineer a wheelchair and rebuild the controls, which will be the main topic of discussion for this paper. Two big challenges with this step are high handicap scooters control and joystick control. An H-bridge motor interface, controlled by a microprocessor, was designed for the motors. The joystick control is handled with the same microprocessor.

Satisfaction with technological equipment in individuals with tetraplegia following spinal cord injury

OBJECTIVE: To measure the acquisition, use and satisfaction with high technology equipment by spinal cord injured tetraplegic subjects. DESIGN AND SETTING: A questionnaire was mailed to 102 tetraplegic subjects who were hospitalised in the rehabilitation center of Kerpape (Ploemeur, France) between 1998 and 2004, and 59 subjects responded. The questionnaire asked about the use of telephones, computers, wheelchairs and environmental controls at home. RESULTS: When a piece of equipment was acquired, it was very often used. Patient satisfaction with equipment was 79.3%. Home phones and mobile phones were often used with options such as hands-free devices (78 and 59% respectively). A total of 64.4% of subjects acquired a manual wheelchair and 61% a power wheelchair. The most commonly acquired options on the power wheelchairs were the powered recline (73,7%) and tilt (71,1%) systems. All options were used but all were more desired than acquired. A total of 27.1% of subjects desired a pushrim-activated handicap scooters, but only 15.3% had acquired one; 695% of subjects had a computer. Communication was the first use for the computer (82.5%); 49.2% of subjects had acquired an environmental control system, but 20% desired one. The first reason for lack of acquisition was financial difficulties but also accessibility and information problems. The factor that influenced the acquisition and need for equipment was the degree of spinal cord injury. No other factor reduced patient satisfaction with equipment. CONCLUSION: Patients were satisfied with the equipment they acquired. But their needs, especially wheelchair options and environmental control systems, were not satisfied.

Our Hawaii “ice storm” is now a decade old and shows no clear sign of ending. Our 4-year-old managed care dual-diagnosis program called Community Care Service is working on this disaster. This drug court, with a census of 180(+), obtains weekly drug screens and ensures daily contact with group therapy. It also adds cognitive “restructuring”, provides transportation to appointments, and monitors via home visits and intensive in-community outreach. Dr. Charles Bogdahn and I are doing a small research study on recovering ice hallucinators in the drug court system.

Alternatives to Incarceration for Drug Users

Another promising alternative to incarceration is the drug court. Supervised by a sitting judge, a drug court is an intensive, community-based alcohol detox malibu, rehabilitation, and supervision program for drug defendants. The drug court movement, which began in Miami in 1989, has now expanded to all 50 States, the District of Columbia, Puerto Rico, Guam, and 2 Federal districts. The Drug Court Survey conducted by the American University Drug Court Clearinghouse reports that, as of October 2000, more than 1,050 drug courts are either operational or in the planning stage. The States with the most drug court programs include California (142), Florida (65), New York (63), Ohio (49), and Oklahoma (32).19

Studies have shown that drug use by participants involved in a drug court program is very low. The percentage of clean drug tests for current drug court participants in a survey of 14 drug courts ranged between 84% and 98%. The percentage of drug court participants who are rearrested while they are in a drug court program is also very low. A national survey of drug courts, sponsored by the U.S. Department of Justice (DOJ), found rearrest rates fell between 2% and 20% for drug court participants. Of those graduates who were rearrested, less than 3% were arrested for a violent offense, and of those, most were misdemeanors. The majority of rearrests were for new drug possession violations or traffic violations.20

Consumer participation in identifying research and development priorities for power wheelchair input devices and controllers

A focus group comprised of persons who use power wheelchairs, handicap scooters and professionals working in the field were asked to participate in a brainstorming session to determine priorities for the development and application of power mobility input devices and control concepts. The group consensus was that durability and reliability are the most important criteria. Essentially, the expectation is that a power wheelchair must work everyday in the way a person needs it and wants it. At the same time, there is a desire to enhance and advance the features of input devices and control systems. Many would say these changes constitute designing “smarter” power wheelchairs, such as systems that can independently detect obstacles and can provide users with more feedback. This paper presents the rationale behind forming this focus group and details of the results of a brainstorming session where ideas were generated and prioritized. The five most important issues as determined by the group are discussed in depth.

Advanced Processing Technology of Electrochromic Windows: A process to develop new film patterning techniques for energy-saving smart windows. The process improves visible light transmission and reduces glare, while retaining a clear view and increasing thermal comfort. Also, encouraging innovation with window discount blinds and shades to help increase energy savings.

An Indirect Route for Ethanol Production: A process to develop fuel ethanol from renewable resources to achieve savings of 80 percent in energy consumption. The process helps U.S. farmers create a market for corn supplies and increases national energy security.

Manufacturing Ceramic Products from Fiberglass Waste: A low-cost energy method of completely transforming fiberglass waste into a dense ceramic product that can be used for window mini blinds, shades, as well as other uses. The process could eliminate the need for disposal of 260,000 tons of fiberglass waste annually.

Near-Infrared (NIR) Veneer Strength Sensor for Forest Industry: Use of NIR sensors instead of ultra-sonic technology allows testing for veneer wood strength before drying. The NIR technology will improve process efficiency and reduce product waste, and is expected to save about $23,000 annually in energy costs.

The Force Modulator: A durable sheet metal technology for the steel industry that allows proper control of press forces during part forming.

What is Cue Reactivity?

About a week and a half ago, I ran a study using a process called cue reactivity. It is a process in which we show individuals stimuli or picture situations that remind them of the alcohol rehab-taking event. There was one fellow who had been dependent for about seven years but was clean for six weeks. We hooked up wires to measure his brain activity, heart, skin temperature and blood pressure and measured his reaction to the stimuli. At the end of the session he said, “You know doctor, I did not feel a thing; you cured me.” When I read the tracings from his data, they were off the scale. That told me he had a visceral response inside his mind. His brain and body are still reacting to the simple sight of someone else using a detox dallas.

It is our requirement as researchers to take this type of information and disseminate it to those who can use it. In a way, this describes a process called “from bench to trench.” Researchers are working on the benches, and my practicing colleagues are working in the trenches. It is important researchers get information to practicing doctors, and we must get it to you.

For more than 20 years, DOE’s I&I program has funded more than 500 energy efficient inventions with nearly 25 percent of them reaching the marketplace. Cumulative sales for those products total nearly $710 million.

DOE’s NICE3 program provides funding to state and industry partnerships (large and small business) for projects that demonstrate advances in energy efficiency and clean production technologies like advanced window blind and window shade systems. State and industry partnerships are eligible to receive a one-time grant of up to $525,000. The industrial partner may receive a maximum of $500,000 in federal funding. The non-federal cost share must be at least 50 percent of the total cost of the project.
